As for tattoos. I have six of the things and I've got to say for me ankle pain (inside and out) was minimal but healing time is an absolute bitch, it takes longer than other places because it's further away from your heart - and I'm not making that up *lol* I found the outline hurts the most, the shading has a tendency to put me to sleep. However a few people reckon it's the other way round - in which case you're in for a ride of discomfort. I love tattoo pain, so I'm totally bias :) My next one will probably be the back of my shoulder (and if not, the outside of my wrist), but it's going to be a biggie - I reckon that'd hurt more than the ankles/lower back.
Some forms of arthritis can be picked up via x-ray, it depends how long it's been there for and all that jazz too :(
Editing to say I just guessed on the two last pain questions, the lower back is a good option but it *does* hurt more the further towards the sides you go. It also depends on the amount of pressure the tattooist puts on you as well, it's really a personal thing you have to gauge.
